You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. All new patients are offered a health check with a member our Nursing team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. This usually takes about 10 minutes.
A doctor will check all the information you have supplied and decide if your registration will be accepted. If for any reason your application is declined (this is a very rare occurrence) we will contact you to explain the reason.
Your registration will be with the practice as a whole rather than with a particular doctor. You will understand that all the doctors are busy and your preferred doctor may be fully booked on the day you wish to see them but we will always endeavour to give you an appointment to suit your needs.
If you move out of the practice area you will need to register with a new practice.
Medical treatment is available from the date of registration.
Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
If required, please contact reception for further information.
It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.
